Anti-TNF medicines for ulcerative colitis

These biologic treatments may be considered when bowel inflammation remains active or other treatment is unsuitable. They can be delivered by an infusion in a clinic or an injection under the skin, depending on the medicine selected.

TNF is an immune-system signal that helps drive inflammation. An anti-TNF medicine attaches to this signal and limits its effect, which can reduce bleeding, urgency, diarrhoea, and abdominal discomfort when those symptoms come from active ulcerative colitis.

Blocking this pathway also changes the body's handling of infection. The gastroenterology team screens for relevant infections before treatment and follows blood results, bowel symptoms, and medicine reactions afterward.

Give the specialist a complete list of medicines and disclose current or recurring infections, previous tuberculosis or hepatitis, heart or neurological problems, cancer treatment, upcoming surgery, pregnancy plans, and earlier biologic use. Vaccination needs should be reviewed before therapy begins.

Attend blood monitoring and bowel assessments even when symptoms improve. For home injections, follow storage, timing, and disposal instructions and note any site reaction. If a dose is missed or an infection develops, seek instructions from the prescribing team rather than adjusting the schedule yourself.

Report fever, chills, a lingering cough, shortness of breath, painful urination, new skin blisters, or a wound that looks infected. Promptly tell the bowel team about more frequent diarrhoea, increasing blood in stool, worsening abdominal pain, night-time bowel movements, or reduced ability to eat and drink.

Call emergency services for throat or facial swelling, severe breathing difficulty, fainting, sudden chest pain, or a major reaction during or after an infusion. Heavy rectal bleeding, severe dehydration, confusion, or intense abdominal pain with swelling also needs emergency assessment.

Discuss the goal of therapy, how response will be measured, whether current bowel medicines continue, and which infection symptoms should delay a dose. Ask about infusion or injection arrangements, vaccinations, travel, pregnancy, bowel surveillance, and the plan if inflammation remains active.
