Anti-tetanus toxin: vaccine, toxoid, or antitoxin?

Tetanus illness is caused by a toxin made by bacteria in contaminated tissue. Tetanus toxoid is an inactivated form used in vaccines to teach the immune system to make its own protection; it cannot cause tetanus.

Tetanus immune globulin, sometimes loosely called antitoxin, supplies ready-made antibodies. It may be considered after certain high-risk wounds when vaccination is incomplete or unknown. It acts differently from the vaccine and does not replace the doses needed for longer-term protection.

Clean an injury under running water and obtain medical advice for a deep, dirty, puncture, bite, burn, or crush wound. Share your vaccine record and any history of a serious reaction to vaccines or antibody products.

The clinician will assess the tissue damage, contamination, time since injury, prior vaccine course, and immune health. Depending on that assessment, care may include wound cleaning, a tetanus-containing vaccine, immune globulin, other treatment, or observation. Do not inject or take a product simply because its label includes “tetanus”; confirm exactly what it is and why it was recommended.

Arrange prompt wound assessment if an object remains embedded, contamination cannot be removed, tissue is badly damaged, or your vaccination history is uncertain. Jaw tightness, difficulty opening the mouth, painful muscle contractions, trouble swallowing, or breathing problems require emergency care.

A spreading red area, worsening swelling, pus, fever, or escalating pain can signal infection in the wound. These changes need clinical review even if tetanus prevention has already been given.

Ask for the medicine’s exact name, whether it is a vaccine or immune globulin, what part of your risk it addresses, and whether follow-up vaccination is needed. Also ask which reactions are expected and which symptoms should prompt immediate help.
