The short answer

It may be planned during pregnancy or offered after an event that can mix fetal and maternal blood, including birth, pregnancy bleeding, abdominal injury, pregnancy loss, or certain procedures. Whether it is needed depends on blood-group results and the event.

What Anti-D prevents

If an RhD-negative person encounters RhD-positive red blood cells, the immune system can form lasting antibodies against them. This sensitisation may affect a later RhD-positive pregnancy because those antibodies can cross the placenta and break down the baby's red blood cells.

Anti-D immunoglobulin removes the triggering cells before the immune system establishes that response. It is preventive rather than a treatment for bleeding, injury, miscarriage, or anaemia, and it cannot remove antibodies once sensitisation has already occurred.

How to use your care plan

Keep your blood-group result with your maternity records and tell a healthcare professional promptly about vaginal bleeding, abdominal trauma, or a procedure during pregnancy. If Anti-D is recommended, attend within the timeframe your maternity team specifies; the dose and route are chosen for the clinical situation and product.

After birth, the team may test the baby's blood group and assess whether another dose is indicated. If you are already sensitised, ask what additional pregnancy monitoring is planned, because routine Anti-D prevention serves a different purpose.

When to contact maternity care

Contact your maternity service promptly after vaginal bleeding, a fall or blow to the abdomen, suspected pregnancy loss, or if you miss a planned Anti-D appointment. Seek emergency help for heavy bleeding, severe abdominal pain, fainting, breathing difficulty, or collapse. The urgency comes from the symptoms or injury, not from how commonly sensitisation occurs.
