Anti-D injection before 12 weeks of pregnancy

Early-pregnancy recommendations vary with the cause, amount and pattern of bleeding, and whether an intervention is planned. Contact your maternity or early-pregnancy service promptly so they can check your blood group and decide whether treatment applies.

Anti-D is relevant when your blood group is RhD-negative and you have not already formed immune anti-D antibodies. Its purpose is to prevent your immune system responding to RhD-positive fetal red blood cells that may enter your circulation.

The team considers whether bleeding is repeated, heavy, or accompanied by pain; whether there has been pregnancy loss or trauma; and whether a procedure enters the uterus. A blood-group result from a previous pregnancy can be helpful, but the current team may confirm it.

Note when bleeding or pain began, whether it is increasing, and whether you passed clots or tissue. Tell the clinician about your gestational stage, blood group if known, previous antibodies, earlier anti-D in this pregnancy, and any planned or completed procedure.

Do not delay an assessment while trying to locate old records. If anti-D is advised, the service will choose the preparation and dose for the event under its protocol. Receiving it does not replace evaluation of the bleeding or pain itself.

Seek urgent care for heavy or rapidly increasing vaginal bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, fever, or feeling acutely unwell. These symptoms need assessment for causes such as significant blood loss or an ectopic pregnancy; the need for anti-D is considered alongside that care.

You can ask whether your RhD status has been confirmed, whether the particular event can expose you to fetal blood, which local recommendation applies before 12 weeks, and what follow-up is needed if bleeding continues or another procedure becomes necessary.
