Anterior Knee Swelling: What It Can Mean

Direct answer
A soft, localised bulge directly over or just below the kneecap differs from swelling that fills the whole joint. Sudden warmth, redness, fever, inability to bear weight or swelling after a major injury needs timely medical evaluation.
How the swelling pattern helps
Repeated kneeling can irritate a small cushioning sac at the front of the knee, a typical presentation of knee bursitis. Swelling within the joint can follow an ACL tear or meniscus tear after twisting, and knee osteoarthritis may produce recurring fullness with stiffness. Gout can present with an abruptly painful, hot and tender knee.
A Baker's cyst forms behind the knee rather than at the front, although the joint problem producing extra fluid may make the entire knee feel swollen. Examination distinguishes a superficial pocket from fluid deeper inside the joint and checks movement, stability, skin and circulation.
Care while arranging assessment
Reduce kneeling, running, squatting or other activity that increases the swelling. Rest with the leg supported and use a wrapped cold pack for comfort after activity or a recent injury. Do not puncture or drain a swelling yourself, because this can introduce infection and does not address the underlying cause.
If swelling does not settle, keeps returning or restricts bending, a clinician may recommend imaging or fluid testing. Joint aspiration may sometimes relieve pressure or help determine whether infection, bleeding or crystals are present.
Warning signs that need care
Seek same-day urgent care for a hot, red, very tender knee, particularly with fever, chills, a wound, drainage or feeling unwell. Infection inside a joint or bursa needs prompt treatment. Urgent assessment is also appropriate when the knee is deformed, locks after injury, cannot take weight, or rapidly becomes very swollen.
Get immediate help if swelling extends into the leg with a cold or discoloured foot, new numbness, chest pain or shortness of breath.
