The term is often used in the same way as antenatal depression. Symptoms can begin at any point before delivery and may affect emotions, thinking, sleep, relationships, physical self-care, and preparation for the baby.
Signs and clinical assessment
A persistent empty or low mood, loss of enjoyment, feelings of worthlessness, excessive guilt, irritability, anxiety, withdrawal, or thoughts of death can form part of antepartum depression. Fatigue, altered appetite, poor sleep, and difficulty concentrating overlap with pregnancy experiences, which is why their severity and effect on functioning matter.
Assessment includes direct questions about safety and support as well as mental and physical health. The clinician may explore previous mood episodes, family history, stressful circumstances, relationship safety, and periods of unusually high energy, impulsivity, or very little need for sleep.
Building a care plan before birth
Book an appointment with a pregnancy-care or primary-care professional and give concrete examples of what has changed. Depending on need, care may include psychological therapy, social support, strategies for daily routines and rest, specialist perinatal mental-health input, or medication.
Prescribing during pregnancy calls for an individual discussion; untreated depression also carries consequences. Continue prescribed treatment until the responsible clinician advises a change, and agree on whom to contact if symptoms worsen. Include plans for the period after delivery because mood and support needs can change then.
Responding to crisis or rapid deterioration
Emergency help is needed for thoughts or plans of suicide, thoughts of harming another person, an inability to stay safe, hallucinations, marked confusion, or fixed unusual beliefs. Contact local emergency services or attend an emergency department, and stay with a trusted adult rather than being alone.
Speak with the maternity or mental-health team promptly when distress escalates, self-care collapses, sleep is severely disrupted alongside agitation or unusual energy, or anxiety prevents essential activities. These changes require timely assessment even without an expressed plan for self-harm.
