How it can feel

Postpartum depression can present with sadness, emptiness, irritability, anxiety, hopelessness, guilt, loss of enjoyment, poor concentration, appetite changes, or difficulty sleeping even when there is a chance to rest. Some people feel detached from their baby or find bonding difficult. Symptoms may start during pregnancy, soon after birth, or later in the first year.

Brief tearfulness and emotional sensitivity soon after delivery are often called the baby blues. When low mood continues, worsens, or interferes with eating, sleeping, self-care, or caring for the baby, an assessment for postpartum depression is appropriate.

Getting support

Tell a doctor, midwife, or perinatal mental health professional how you have been feeling and how this affects daily tasks. They may ask about mood, anxiety, sleep, safety, previous mental health problems, and available support. Treatment can include talking therapy, practical support, and medication selected with feeding plans and personal preferences in mind.

While arranging care, ask a trusted person to help with meals, household work, baby care, and opportunities to sleep. Avoid relying on alcohol or non-prescribed drugs to manage distress, because they can worsen mood and reduce safety.

When to seek urgent care

Get immediate emergency help if you might harm yourself or the baby, cannot keep either of you safe, or have thoughts of suicide. Sudden confusion, extreme agitation, hearing or seeing things others do not, fixed unusual beliefs, or a striking loss of contact with reality may indicate postpartum psychosis and require emergency assessment.

Do not stay alone while waiting for urgent help. Ask a trusted adult to remain with you and care for the baby, and call local emergency services or go to the nearest emergency department.

You may want to ask which treatment fits your symptoms, how breastfeeding or other feeding choices affect medication decisions, who to contact if your mood worsens, and how family members can support your care. Mention any past depression, bipolar disorder, psychosis, trauma, or difficult reaction to psychiatric medicine.
