What to know about misoprostol

In women's healthcare, clinicians may use it as part of medical management of miscarriage or to induce labour. The dose, route, timing, and place of treatment depend on the reason it is prescribed and the person's clinical circumstances.

The medicine acts in a similar way to prostaglandins, substances involved in cervical softening and uterine contractions. Cramping and vaginal bleeding can therefore be expected when it is used for miscarriage care. During induction, the maternity team monitors contractions and the baby's wellbeing because the response varies between people.

Misoprostol is not interchangeable across all situations. A regimen intended for one purpose should not be reused for another, and tablets supplied to someone else may have unsuitable instructions or strength.

Take or insert the medicine exactly as your prescribing team directs. Confirm where to place the tablets, when to take each dose, which pain relief is suitable, and whom to contact if the expected process does not begin. Tell the clinician about previous caesarean birth or uterine surgery, current medicines, allergies, and any possibility of pregnancy outside the planned treatment.

Keep follow-up appointments after miscarriage treatment so the team can check whether the pregnancy tissue has passed. Do not share remaining tablets or change the schedule without clinical advice.

Use the urgent contact instructions supplied by your clinic if bleeding is very heavy, pain is severe or worsening despite recommended relief, you feel faint, or you develop fever, chills, unpleasant-smelling discharge, breathing difficulty, facial swelling, or a widespread rash. During labour induction, immediately alert the maternity team to constant abdominal pain or any concern about movement from the baby.

Ask what outcome to expect, how cramping and bleeding will be managed, which symptoms need a same-day call, and how completion will be confirmed. If misoprostol is being used for induction, ask what monitoring is planned and what alternatives may be considered if labour does not progress as intended.
