Recent twisting, impact, skin injury and joint inflammation offer useful clues, but a blood clot or infection must be considered when warning signs are present.
Why one side changes the assessment
A sprained ankle commonly presents with swelling, bruising and tenderness after a twist. An ankle fracture can present with marked swelling, difficulty bearing weight and focal bone tenderness after trauma. Gout commonly presents with abrupt joint pain, heat, redness and swelling. Longer-standing one-sided puffiness can relate to previous injury or impaired vein or lymph drainage. None of these patterns proves the cause; the timing, examination and sometimes imaging or other tests distinguish them.
What you can check now
Look for a recent twist, bite, cut or pressure from footwear. Compare ankle size and skin colour, then notice whether swelling reaches the calf or leaves an indentation when pressed. If a mild injury is the clear trigger and you can walk safely, protect the ankle, reduce aggravating activity, elevate it and use a wrapped cold pack. Do not massage sudden unexplained swelling. Arrange medical review when there is no clear cause, walking is difficult, bone is tender, or symptoms are not improving as expected.
Reasons to seek urgent help
Urgent assessment is appropriate when one-sided swelling begins suddenly without an obvious minor injury, spreads into the calf, or comes with warmth, redness, significant pain or fever. Seek emergency help if chest pain, shortness of breath, coughing blood, faintness or confusion develops. After trauma, urgent care is also needed for a deformed ankle, an open wound, inability to take steps, numbness, or a foot that becomes unusually pale or cold.
What to ask during assessment
During the visit, clarify which finding explains the one-sided pattern, whether the calf and circulation need evaluation, and whether an X-ray or another test is indicated. If an injury is diagnosed, discuss how much weight the ankle can take, when movement should restart, and which symptoms would mean returning sooner.

Does one swollen ankle always mean a blood clot?
No. Injuries, infection, joint inflammation and drainage problems can also affect one side. Because a clot cannot be excluded by appearance alone, sudden unexplained swelling deserves urgent assessment.
Can gout affect only one ankle?
Yes. Gout can present in a single joint with sudden severe pain, heat, redness and swelling. A clinician may need to distinguish it from infection or another inflammatory joint problem.
