What to do first
If a minor injury is likely, protect the ankle from aggravating activity, elevate it, and use a wrapped cold pack for short comfortable intervals. Supportive footwear may improve confidence. Begin gentle ankle movement as comfort allows, but stop if it causes sharp pain or the ankle feels unstable.
For unexplained swelling, compare both sides and note onset, warmth, skin colour, tenderness, and changes through the day. Avoid deep massage when a clot has not been excluded. Do not rely on bruising to decide whether walking, sport, or work duties are safe.
When the ankle needs assessment
Seek prompt care if you cannot bear weight after an injury, the ankle is deformed, bone is sharply tender, or swelling is severe or worsening. Medical review is also needed for unexplained one-sided swelling, a hot red area, fever, an open wound, numbness, or a foot that becomes unusually pale or cool.
Call emergency services if leg swelling occurs with sudden breathlessness, chest pain, coughing blood, collapse, or severe lightheadedness. These symptoms can indicate a problem beyond the ankle and should not wait for bruising or pain to develop.
Questions for the examination
Ask whether the location of tenderness suggests ligament, tendon, joint, or bone injury; whether imaging is warranted; and when weight-bearing and rehabilitation can progress.
