Ankle swelling and kidney disease: what patients should know

The swelling often affects both ankles and may occur with puffiness elsewhere, a change in weight, or altered urination. Its presence does not show how well the kidneys are working, so a clinician may use an examination and tests to assess the cause.

Kidneys help regulate salt and water. When that regulation is impaired, extra fluid can remain in the circulation and move into tissues, becoming visible around the ankles under the influence of gravity. Loss of protein through the kidneys can also contribute to tissue swelling.

Not every swollen ankle in a person with kidney disease comes from the kidneys. Vein problems, limited movement, heart or liver conditions, medicines, inflammation, and a clot remain possible. A hot, painful joint is a different pattern from soft swelling across both lower legs.

Follow the salt, fluid, medicine, and monitoring plan provided by the kidney team. Record whether swelling affects one or both sides, when it began, whether it reaches the calves, and whether shoes or socks leave deeper marks. Track weight and urine changes if that is already part of your care plan.

Do not take an extra diuretic, change a prescription, or sharply alter fluid intake without guidance. Elevating the legs may ease pooling, and gentle movement can help if it is safe for your mobility. Protect stretched skin from friction and report weeping, cracks, or wounds.

Seek emergency care if swelling accompanies severe breathlessness, chest pain, faintness, confusion, or coughing blood. Sudden one-sided leg swelling with calf pain, warmth, or colour change also needs urgent evaluation.

Contact the kidney care team promptly for worsening swelling, much less urine than usual, new breathlessness, rapid weight change, facial puffiness, or difficulty following the prescribed fluid plan. Fever, spreading redness, broken skin, or a very painful joint requires timely clinical review as well.
