What should you know about ankle swelling and bruising without injury?

A bruise forms when blood leaks from small vessels under the skin, while swelling reflects fluid collecting in tissue. Minor pressure or a knock you did not notice can cause both, but unexplained changes can also relate to fragile skin, inflammation, vein problems, a bleeding tendency, or medicines that affect clotting. Sun-damaged skin may bruise more readily because its supporting tissue is fragile.

The pattern guides urgency. Clinicians consider whether one or both ankles are involved, how quickly the swelling appeared, whether the area is hot or tender, and whether bruises occur elsewhere. This symptom combination does not identify a single diagnosis on its own.

Note when the swelling began, whether it changes after rest, and whether walking causes pain. Take a clear photograph, compare both ankles, and check for warmth, redness, broken skin, calf tenderness, or additional bruises. Review any recent medicine or supplement changes, but do not stop a prescribed blood thinner without medical instructions.

Until assessed, protect the bruised area from pressure and avoid vigorous massage. Resting with the ankle comfortably supported may reduce discomfort, provided this does not delay urgent care. Do not apply tight compression when the cause is unknown or when the foot is numb, unusually pale, cold, or very painful.

Seek urgent medical help for sudden swelling in one leg, especially with calf pain, warmth, or redness. Chest pain, breathlessness, coughing blood, fainting, or severe weakness requires emergency care. A cold or pale foot, intense pain, rapidly increasing swelling, fever, or spreading redness also needs prompt assessment.

Arrange timely review if the swelling persists, returns, or occurs with frequent unexplained bruises, nosebleeds, bleeding gums, blood in urine or stool, or unusually heavy menstrual bleeding. Mention liver or kidney problems and all medicines and supplements, because this information can change which examinations or tests are appropriate.

Ask whether the swelling appears local or related to circulation, whether your medicines could contribute to bruising, and whether blood or imaging tests are needed. Share the timing of recent travel, reduced movement, illness, procedures, new exercise, and any family or personal history of unusual bleeding or clots.
