Short answer

Ankle osteochondritis dissecans is a joint lesion in which underlying bone and its cartilage covering can crack, loosen or separate.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Affected tissue
Bone beneath the joint surface and its cartilage covering
Possible symptoms
Activity-related pain, swelling, stiffness, catching or locking
Assessment
Clinical examination supported by appropriate imaging

Direct answer

It may cause activity-related ankle pain, swelling, stiffness, or a catching sensation. Assessment usually combines an examination with imaging to define the lesion and whether it remains stable.

What the diagnosis means

The lesion involves a small area beneath the smooth cartilage at the ankle joint surface. Repeated loading can irritate it; a loose fragment may interfere with movement and produce catching or locking. Symptoms alone cannot show the lesion's size or stability, so clinicians may use an X-ray and sometimes more detailed imaging.

Management depends on skeletal maturity, symptoms, lesion position, stability and response to activity changes. A stable lesion may be managed without an operation, while an unstable or detached fragment may need an orthopedic surgical opinion.

What you can do now

Pause running, jumping and sport that provokes pain, and avoid repeatedly testing the ankle through painful impact. Arrange an assessment if pain or swelling persists, returns with activity, or limits walking. A clinician can advise whether temporary protection with a brace, boot or crutches is appropriate.

Rehabilitation is usually staged. It may first protect the joint, then restore comfortable ankle movement, strength, balance and controlled loading. Return to sport should follow clinical guidance and evidence of healing rather than pain tolerance alone. Cartilage repair surgery is considered only when the lesion's features and symptoms make an operation appropriate.

When to seek care

Seek prompt medical assessment if the ankle locks and cannot move normally, you suddenly cannot bear weight, or pain and swelling follow a significant injury. Urgent care is also appropriate for a hot, red, rapidly swelling joint, fever with ankle symptoms, a visibly deformed ankle, or a foot that becomes numb, pale or cold.

Book a routine appointment for recurring deep ankle pain, repeated swelling after activity, catching, or reduced motion that does not settle with rest.

Questions for your doctor

Ask where the lesion is, whether it appears stable, which activities to avoid, and what signs of healing will guide progression. You can also ask whether repeat imaging is needed, what rehabilitation should target, and which findings would lead to a surgical discussion.

Questions people ask

Can ankle osteochondritis dissecans heal without surgery?

Some stable lesions can heal with protected activity and monitored rehabilitation, particularly while the skeleton is still growing. The decision depends on imaging, symptoms and progress over time.

Why does the ankle catch or lock?

An irregular joint surface or a loose bone-and-cartilage fragment can obstruct smooth movement. New or persistent locking warrants medical assessment.

Can I exercise with the condition?

Low-impact movement may be suitable, but painful impact and twisting should be paused until a clinician defines safe loading. A physiotherapist can tailor progression to the lesion and your symptoms.
