What should you know about ankle exercises after sprain?

Short answer

After an ankle sprain, begin with gentle pain-limited movement and progress to strength and balance exercises as swelling settles and walking becomes more comfortable and controlled.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Early goal
Restore comfortable ankle motion and a smooth walking pattern.
Later goal
Rebuild calf strength, balance, and control during direction changes.
Stop signal
Sharp pain, increasing instability, or worsening session-to-session swelling needs reassessment.

What exercise is addressing

A sprain stretches or tears ankle ligaments and can temporarily reduce motion, muscle output, and the joint's position sense. That loss of position sense may make the ankle feel unsteady even after the initial soreness improves. Rehabilitation therefore needs more than flexibility: it rebuilds control for uneven ground, turns, stairs, and changes of direction.

The starting level depends on the injury. Mild swelling with a near-normal walking pattern differs from marked bruising, focal bone tenderness, or inability to take steps. If the original injury was never assessed and those more severe features are present, obtain an examination before treating it as a routine sprain.

How to build back control

Start by moving the foot up and down and making small circles within a range that does not create sharp pain. As weight-bearing becomes comfortable, practise even standing with both feet and a smooth heel-to-toe walking pattern. Avoid limping through longer distances simply to increase activity.

Next, add controlled calf raises while holding a stable surface and gentle resistance in different ankle directions. Balance can begin with supported standing on the injured leg, then progress by reducing hand support or adding safe reaching tasks. Sport-specific drills belong later, after everyday walking, calf strength, and single-leg control have returned without the ankle giving way.

A small symptom response can occur, but the ankle should not become progressively more painful, swollen, or unstable from one session to the next. Reduce the range or load if it does. Keep notes on the exercise performed and the ankle's later response so that progression decisions are based on a clear pattern. Physiotherapy can tailor the sequence when you have repeated sprains, persistent stiffness, poor balance, or uncertainty about technique.

Signs that need assessment

Seek prompt care if you cannot bear weight, there is deformity or focal tenderness over an ankle or foot bone, or pain and swelling are severe after the injury. Urgent assessment is needed if the foot becomes cold, pale, blue, numb, or weak, or if redness and warmth spread with fever.

Book a review if the ankle repeatedly gives way, becomes locked, remains significantly swollen, or is not regaining motion and walking function. A new snap, sudden loss of push-off strength, or sharp pain during rehabilitation should also halt exercise until the injury is reassessed.

Questions about your recovery plan

Ask which ligament pattern is suspected, whether you need protection with a brace or splint, and how to judge an acceptable response to loading. For return to sport, ask which strength, balance, hopping, and direction-change tasks you should demonstrate before full training.

Questions people ask

Should I keep exercising if the ankle is still swollen?

Some swelling can remain while gentle rehabilitation begins, but exercise intensity should be reduced if swelling steadily increases or function worsens. Marked or unexplained swelling merits assessment.

Are ankle circles enough after a sprain?

Circles can help restore motion, but they do not fully retrain strength or position sense. A complete progression also includes loading, calf work, balance, and tasks matched to your activities.

Can taping replace ankle exercises?

Taping may provide temporary support or feedback for selected people, but it does not replace graded work on motion, strength, balance, and movement control.
