Angular cheilitis and dry mouth

Dryness can make lips easier to split, while saliva pooling in a deep mouth fold can keep the corner damp and irritated. The result may be soreness, burning, redness, crusting or a fissure that hurts when the mouth opens. Treating only dryness may not clear a fungal or bacterial complication.

Saliva normally lubricates the mouth and helps control its microbial environment. Reduced flow can leave the lips dry and may be associated with oral thrush, while repeated licking exposes the mouth corners to moisture and digestive enzymes. Medicines, dehydration, mouth breathing and conditions that affect saliva production are possible contributors. Ill-fitting dentures or a changed bite may deepen the corner folds. A clinician may inspect the tongue, inner cheeks, teeth, dentures and skin rather than treating the fissure in isolation.

Apply a plain, fragrance-free barrier ointment to clean, dry mouth corners and resist licking or picking. Sip water regularly if fluids are not restricted, use sugar-free gum or lozenges if safe for you, and avoid alcohol-containing mouthwash when it stings or worsens dryness. Keep dentures clean and remove them as advised overnight. Do not stop a prescribed medicine because it may cause dry mouth; a pharmacist or prescriber can review alternatives and saliva-support products. Antifungal or antibiotic treatment should match the suspected cause, and steroid creams around the mouth should be used only with professional guidance.

Book an appointment if cracks persist, repeatedly return, spread beyond the corners, ooze, develop thick crusts, or occur with white mouth patches, dental trouble, difficulty eating, marked thirst or dry eyes. Seek urgent care for rapidly increasing facial swelling, severe pain, fever with spreading redness, trouble swallowing, or breathing difficulty. Persistent dry mouth also deserves attention because it can increase tooth decay and oral infection risk.

Ask whether the corner changes look irritant, fungal, bacterial or eczematous; whether a mouth or skin swab would change treatment; whether medicines or dentures are worsening dryness; and whether tests for iron, vitamin deficiencies, blood glucose or a saliva-gland condition fit your wider symptoms. Bring a current medicine list and describe when the dryness began.
