Angular cheilitis: sore cracks at the mouth corners

The spelling “angul r cheilitis” usually refers to angular cheilitis. Saliva, irritation and infection can overlap, so treatment is chosen according to the likely contributors rather than appearance alone.

Moisture collecting in the folds at the mouth corners can soften and damage the skin barrier. Lip licking, drooling, dry or sensitive skin, dental appliances and changes in bite can increase irritation. Yeast or bacteria may then grow in the damaged area.

Persistent or recurrent cases can be associated with nutritional deficiency, diabetes, immune problems or another skin condition. A cold sore more often begins with grouped blisters and tingling, while angular cheilitis centres on the corner fissure; a clinician can distinguish uncertain cases.

Keep the corners gently clean and dry, avoid licking or picking, and apply a bland protective emollient. Check whether toothpaste, cosmetics or a poorly fitting dental appliance repeatedly irritates the area. Avoid sharing lip products while the skin is cracked.

A clinician may prescribe an antifungal or antibacterial treatment when infection is suspected. A topical corticosteroid may occasionally be used briefly for significant inflammation, usually with attention to any infection; unsupervised steroid use can mask or worsen some infections. Recurrent disease may prompt checks for an underlying contributor.

Arrange assessment if the area is spreading, producing pus, becoming increasingly painful, failing to settle with barrier care, or returning repeatedly. Seek prompt help for fever with rapidly spreading facial redness, marked swelling, trouble opening the mouth, or difficulty swallowing or breathing. People with diabetes or reduced immunity should seek advice early when signs of infection appear.

Ask whether yeast, bacteria, irritation or another skin disorder is suspected; how each prescribed cream should be applied; and whether dental fit or nutritional health needs review. Mention recurrences, dry mouth, denture use, dietary restriction, diabetes and any immune-suppressing treatment.
