Angioedema linked to blood pressure medication

Angioedema is sudden swelling in deeper layers beneath the skin. Certain medicines, particularly ACE inhibitors, can trigger swelling even after they have been taken uneventfully for a long period. The reaction may occur without itching or hives.

Medicine-related angioedema often affects the lips, tongue, face or throat. Hands, feet, genitals or the bowel can also be involved; bowel swelling may cause strong abdominal pain, nausea or vomiting. ACE-inhibitor reactions involve accumulation of bradykinin rather than a typical histamine allergy, so hives may be absent and standard allergy medicines may have limited effect.

The medication name and timing matter, but timing alone cannot rule the medicine in or out. A clinician also considers foods, other medicines, infection, hereditary forms and episodes with no identifiable trigger.

For new swelling, obtain urgent medical advice and provide the exact name of every medicine you take. Bring the packet or a clear photograph of its label. If a clinician suspects an ACE inhibitor, it is generally stopped and should not be restarted; another blood pressure plan must be selected for you.

Do not replace the medicine yourself or use a relative's tablets. Record the reaction in your health records and tell future prescribers and pharmacists. Blood pressure still needs management, so arrange follow-up to choose a suitable alternative and check that control remains adequate.
