How much spotting can occur in early pregnancy?

It may appear as a few pink, red or brown marks on underwear or toilet tissue and does not soak a sanitary pad. Bleeding that fills a pad, keeps increasing or includes clots is more than spotting and needs prompt clinical advice.

Light bleeding can occur when the cervix is more sensitive in pregnancy, including after sex or an examination. Infection, bleeding around the pregnancy, early pregnancy loss and ectopic pregnancy can also present with vaginal bleeding. Colour cannot confirm the source: brown blood is often older blood, while fresh bleeding is usually red.

The timing, trend and accompanying symptoms give the clinician more useful information than volume alone. They may ask about your last menstrual period, previous pregnancies and pain, then recommend an examination, ultrasound or blood tests based on the situation.

Use a sanitary pad rather than a tampon so you can monitor the bleeding. Record when it began, whether it is increasing, its colour, and whether there are clots, pain, dizziness, fever or unusual discharge. Contact your maternity team or healthcare professional the same day for guidance, even if the spotting has stopped, particularly if your pregnancy has not yet been confirmed inside the uterus by ultrasound.

Avoid self-treating with medicine left from another illness. If you know your blood group is rhesus negative, mention this when seeking advice because the team will decide whether any additional care is appropriate.

Go to an emergency department or call emergency services for heavy or rapidly increasing bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, weakness, breathing difficulty or collapse. Ectopic pregnancy can present with vaginal bleeding and these symptoms, and it requires urgent assessment.

Prompt assessment is also needed for bleeding with fever, chills, worsening cramps, passing tissue or feeling very unwell. Use symptoms and bleeding changes—not assumptions about how common a cause may be—to decide how urgently to seek help.

You may want to ask whether the pregnancy location has been confirmed, what the examination or scan showed, and whether follow-up blood tests or another ultrasound are needed. Clarify which changes in bleeding or pain require immediate return, which medicines are suitable for you, and who to contact if symptoms recur outside clinic hours.
