Mesothelioma information: diagnosis, care, and next steps

Mesothelioma is a cancer of the mesothelium, the thin tissue that lines body cavities and covers certain organs. It most often develops in the lining around the lungs, called the pleura, and can also arise in the lining of the abdomen, called the peritoneum.

Pleural mesothelioma can present with shortness of breath, cough, pain under the ribs, fatigue, or fluid around a lung. Peritoneal mesothelioma can present with abdominal pain or swelling, constipation, fatigue, or unexplained weight loss. Other conditions can cause the same symptoms, so symptoms alone do not confirm mesothelioma.

Diagnosis generally requires imaging and examination of cells or tissue. A pathologist may use immunohistochemistry to distinguish mesothelioma from cancers that can look similar. The pathology subtype, where the tumour is located, its extent, fluid build-up, and a person's heart, lung, and general health all shape treatment planning.

Past asbestos exposure is an important risk factor. Tell your clinician about work, home renovation, military, construction, shipyard, industrial, or household contact that may have involved asbestos, even if the exposure occurred long before symptoms began. Exposure history supports the assessment but does not replace a biopsy diagnosis.

Ask for the exact tumour location, pathology subtype, and stage, along with an explanation of whether more imaging or tissue review is needed. Because mesothelioma care can involve several specialties, the team may include medical and radiation oncologists, thoracic or abdominal surgeons, respiratory clinicians, pathologists, and symptom-control specialists.

Treatment may include surgery for selected tumours, chemotherapy, immunotherapy, radiation therapy, or a combination. The choice depends on the tumour site and extent, whether surgery is feasible, previous treatment, other medical conditions, expected side effects, and your priorities. A second pathology or specialist opinion can be useful when the diagnosis or options remain unclear.

Palliative care can begin alongside cancer-directed treatment. It focuses on breathlessness, pain, fatigue, appetite, sleep, emotional strain, and practical support. Procedures to drain fluid from the chest or abdomen may also relieve pressure and improve comfort when fluid accumulation is causing symptoms.

Arrange a medical assessment for persistent or worsening shortness of breath, cough, pain beneath the ribs, abdominal swelling, unexplained weight loss, or marked fatigue. Mention any known asbestos exposure and how the symptoms have changed. These features have many possible causes, and assessment is needed to identify the cause.

Seek urgent care for sudden or severe difficulty breathing, chest pain, coughing up blood, fainting, new confusion, or blue or grey lips. If you are receiving treatment, also use the oncology team's urgent contact instructions for fever, uncontrolled vomiting, inability to keep fluids down, or rapidly worsening pain.

Consider asking: Where did the mesothelioma start, and what is its subtype and stage? Is more tissue testing needed? What is the goal of each treatment option? How might treatment affect breathing and daily activity? Who should I contact for new symptoms, and which supportive-care services are available now?
