Because a reaction can be serious, it belongs in a clinic equipped to observe symptoms and treat anaphylaxis. It should not be attempted at home unless an allergy specialist has explicitly provided a plan for that situation.

What the challenge can clarify

Skin or blood allergy tests can show sensitisation, but they do not always prove that eating the food causes symptoms. A supervised oral challenge observes what happens after the person eats gradually increasing portions of the suspected food under controlled conditions.

An allergy specialist may use the result to confirm an allergy, check whether a known allergy has resolved, or decide whether a food can return to the diet. The clinician interprets symptoms, examination findings and testing together rather than relying on one result alone.

How to prepare and what happens

Follow the clinic's instructions about medicines, eating beforehand and which food to bring. Do not stop an antihistamine, asthma medicine or other prescribed treatment unless the clinical team directs you. Tell them about recent fever, cough, wheeze, rash or an asthma flare because illness may affect safety or interpretation.

At the appointment, staff check baseline health, give measured portions at planned intervals and watch for changes in the skin, mouth, throat, stomach and breathing. Observation continues after the final portion. Plan enough time for monitoring and arrange transport if the clinic advises it.

If symptoms develop

Report any itching, hives, throat discomfort, vomiting, cough, voice change, wheeze, dizziness or unusual sensation immediately during the challenge. The team may pause or stop the test, examine you and give treatment based on the reaction.

After leaving, follow the discharge instructions. Use prescribed adrenaline immediately and call emergency services if symptoms match your anaphylaxis action plan, particularly breathing difficulty, throat or tongue swelling, collapse, or rapidly progressing symptoms affecting more than one body system.

Confirm why the challenge is recommended, what result would stop it, which medicines need adjustment, how asthma will be assessed, and what to eat afterward. Also ask what a negative or inconclusive result means for future portions and emergency medication.
