Allergic reaction and diaper rash: what parents should know

Frequent gentle changes, careful drying, diaper-free time, and a thick fragrance-free barrier ointment may help. Remove newly introduced products, and have a clinician assess severe, spreading, unusual, or non-improving inflammation.

Irritant diaper dermatitis usually affects skin that has prolonged contact with urine or stool and may spare deeper creases. Allergic contact dermatitis can begin after a change in wipes, cleanser, cream, laundry product, or diaper brand and may follow where that material touches.

A bright rash involving the folds with small nearby spots may indicate yeast overgrowth. Blisters, open sores, honey-coloured crusts, or pus raise concern for another infection. Appearance alone can be misleading, so avoid treating every diaper rash as allergy or fungus.

Change wet or soiled diapers promptly. Clean with lukewarm water and soft cotton, pat dry, and allow air time when practical. Apply a generous layer of simple barrier ointment; remove only soiled material at the next change instead of rubbing all the ointment away.

Pause scented wipes, soaps, powders, botanical oils, and recently added creams. A pharmacist or clinician can advise whether an emollient, antifungal treatment, or mild anti-inflammatory medicine matches the pattern. Do not apply adult-strength steroid, combination cream, antiseptic, or medicated powder without paediatric advice.

Seek prompt care if your baby is feverish, unusually sleepy, feeding poorly, in marked pain, or has rapidly spreading redness, significant swelling, blisters, ulcers, pus, or crusting. A very young infant who seems unwell should be assessed urgently.

Arrange an appointment if careful barrier care does not improve the eruption, if it keeps returning, or if you suspect a specific product. Emergency help is needed for breathing difficulty or swelling of the lips, tongue, or face.

Bring the names or packaging of recent products and ask whether the distribution suggests irritation, contact allergy, yeast, or bacterial infection. Confirm which barrier product to use and where, how often, and how long any medicated cream should be applied.
