The short answer

Heat rash, also called miliaria or prickly heat, develops when sweat becomes trapped in the skin. Hives triggered by warmth or sweating are raised, itchy welts caused by histamine release and may move, fade, or change shape.

How to tell the patterns apart

Heat rash often presents with clusters of tiny bumps, prickling, redness, or itch in sweaty or covered areas such as the neck, upper chest, back, skin folds, or beneath tight clothing. The bumps tend to stay in the same area until the skin cools and dries.

Heat-triggered hives are usually smooth, raised welts rather than fixed pinhead bumps. A reaction to sunscreen, fragrance, fabric, or laundry products may instead follow the exact area of skin contact. Appearance alone can be misleading, especially on deeper skin tones where redness may be subtle.

What you can do now

Move to a cool place, remove damp or tight clothing, take a cool shower, and let the skin air-dry. Choose loose, breathable clothing and pause heavy exercise until the irritation settles. Avoid scratching, hot showers, fragranced products, and thick greasy ointments that can trap heat and sweat.

A light, fragrance-free moisturiser may help if the skin is dry, but treatment depends on the cause. A pharmacist or clinician can advise whether an antihistamine suits hive-like welts or whether a short course of a suitable topical corticosteroid is appropriate for inflammation. Do not apply steroid creams to broken skin or sensitive areas unless specifically advised.

When to seek medical care

Get emergency help for difficulty breathing, wheezing, faintness, throat tightness, or swelling of the lips, tongue, or face. These signs can accompany a serious allergic reaction and should not be managed as ordinary heat rash.

Arrange a medical assessment if the rash is widespread, keeps returning with heat or exercise, does not improve after cooling, or is becoming more painful. Increasing warmth, swelling, pus, crusting, fever, or red streaks can suggest infection and also need prompt attention.
