Allergic Contact Dermatitis From Nickel

The area may itch, redden, swell, blister, crack or become dry. Avoiding further contact is central to recovery, while suitable skin care and prescribed treatment can settle inflammation.

The immune reaction is confined mainly to skin that encounters nickel, although inflammation can extend beyond the exact contact outline. Common exposure points include earlobes beneath earrings, wrists under watches, the lower abdomen near a metal fastener, and fingers handling metal objects.

A reaction may emerge after repeated exposure even if an item caused no trouble before. Sweat, friction and damaged skin can make contact more irritating. A clinician may consider patch testing when the trigger is uncertain or the rash repeatedly returns in the same pattern.

Remove the suspected item and avoid testing it repeatedly on inflamed skin. Check jewellery, watch backs, belt buckles, clothing fasteners, keys, tools and phone accessories that touch the affected area. Choose clearly labelled nickel-free alternatives or place a durable barrier between the metal and skin.

Wash gently, pat dry and use a fragrance-free moisturiser to support the skin barrier. Cool compresses can ease itching. Avoid scratching and fragranced products on broken skin. A clinician may recommend a topical corticosteroid of suitable strength for the body site and severity; delicate areas need particular care.

Arrange an assessment if the eruption is widespread, keeps returning, affects the face or genitals, interferes with sleep, or does not improve after the suspected metal is removed. Increasing pain, warmth, pus, rapidly spreading redness or fever can suggest infection and merit prompt care.

Seek urgent help for swelling of the lips or tongue, breathing difficulty, faintness or a rapidly developing widespread reaction. Those features are not the typical local pattern of nickel contact dermatitis and need immediate evaluation.

Ask whether the distribution fits nickel allergy, whether patch testing would clarify the trigger, and which moisturiser or medicine is suitable for the affected site. It can also help to ask how long to use treatment and which signs should prompt reassessment.
