Allergic contact cheilitis of the lips

Possible signs include redness, flaking, fissures, itch, tenderness, or burning across the lip surface and border. Because this immune response is delayed, the connection with a lipstick, balm, toothpaste, flavouring, dental item, or hand-transferred product may be easy to miss.

A trigger may touch the lips directly through cosmetics, oral-care products, topical medicines, food contact, or occupational items. Indirect exposure can come from fingers, musical instruments, dental appliances, or close contact with another person's product. The product name alone is not enough because formulas and ingredient names vary.

Irritant damage from saliva, weather, rubbing, or strong oral-care ingredients may produce nearly the same appearance. Infection and other inflammatory lip conditions can also coexist, so recurrent symptoms deserve a diagnosis rather than repeated trial-and-error treatment.

Make a complete inventory of anything contacting the mouth, including products used only occasionally. Note brand, shade or flavour, start date, and whether symptoms ease when it is absent. Simplify the routine to tolerated essentials and protect the surface with a bland ointment rather than repeatedly sampling new balms.

A dermatologist may perform patch testing with a standard allergen series, extra substances relevant to lip exposure, and selected personal products. A positive result must match real-world contact before it explains the rash. Once identified, the allergen's alternative names should be checked on ingredient lists.

Emergency help is needed for rapidly developing lip or facial swelling accompanied by breathing difficulty, throat tightness, dizziness, or widespread hives. Delayed contact cheilitis usually stays at exposed skin, so those sudden systemic symptoms require a different level of response.

Book an earlier examination if crusts spread, pain becomes marked, fever appears, or drinking is difficult. Unexplained ulcers, a persistent lump, ongoing one-sided change, or failure to improve after removing suspected products also needs direct assessment.

Bring photographs from flare days and ask which personal products are suitable for patch testing, how to prepare for the test, and how results will be matched to exposures. Request a written list of ingredient synonyms if an allergen is confirmed.
