What you need to know

Most useful clues are whether a primary rash is present, which areas itch most, when symptoms worsen, and whether urine, stool colour, skin colour or fetal movement has changed.

Why the presence of a rash matters

Dry scaling or recurrent inflamed patches can fit eczema, while temporary raised welts are a typical presentation of hives. Contact dermatitis often appears where a new product, adhesive or fabric touched the skin. Scabies usually presents with marked itch, often worse at night, and may affect close contacts.

A pregnancy-related liver disorder can cause strong itching without an initial rash, particularly on the hands and feet. Only scratch marks may be visible. Clinical assessment and, when indicated, blood testing help distinguish this from a skin eruption.

How to respond and prepare

Call the maternity service and explain whether the itching is new, widespread, sleep-disrupting or concentrated on the palms and soles. Note when it began and photograph any rash before applying products.

For temporary comfort, pat rather than scratch, keep nails smooth, wear soft loose layers and apply a plain fragrance-free emollient. Use cool or lukewarm water. A pharmacist or maternity clinician can check whether a proposed cream or oral medicine is appropriate during pregnancy.

When not to wait

Obtain urgent maternity assessment if the itch is severe, keeps worsening, has no obvious rash, or prominently affects the palms or soles. Yellow eyes or skin, dark urine, pale bowel movements, nausea with upper abdominal pain, or reduced fetal movement add urgency.

Use emergency services for throat or tongue swelling, breathing difficulty, collapse, or a widespread blistering or peeling rash. A hot painful rash with fever also warrants urgent examination.
