Blood pressure medicines: types, safe use and side effects

Some people use one medicine, while others need medicines with complementary actions. Treatment aims to reduce strain on blood vessels and organs without causing troublesome low pressure or other adverse effects. Never stop, double or swap a prescribed dose on your own.

ACE inhibitors and angiotensin receptor blockers relax blood vessels through related hormone pathways. Calcium-channel blockers reduce tightening in artery walls. Thiazide-like diuretics help the kidneys remove sodium and water, while beta blockers slow the heart and reduce its workload in selected situations. Each group has different cautions, interactions and monitoring needs.

Hypertension often causes no symptoms, so feeling well does not show whether treatment is working. Headache alone also cannot confirm high blood pressure; a properly taken measurement is needed. Home or ambulatory readings may help separate a sustained rise from changes linked to the clinical setting or daily activity.

Take each dose at the agreed time and use a pill organiser or reminder if helpful. Record home readings only as instructed: sit quietly, support the arm and use a correctly fitting cuff. Bring the readings and an up-to-date medicine list to follow-up visits.

Tell the prescriber about dizziness on standing, ankle swelling, cough, unusual tiredness or changes in pulse. Also mention painkillers, cold remedies, supplements and herbal products because some can alter blood pressure, kidney function or treatment effects. If you are pregnant, planning pregnancy, vomiting, having diarrhoea or unable to drink normally, obtain specific advice about which medicines to continue.

Call emergency services for new chest pressure, severe breathlessness, collapse, or stroke signs such as a suddenly drooping face, arm weakness or speech difficulty. Sudden swelling of the lips, tongue or throat, wheezing or trouble breathing after a medicine may be a severe allergic reaction and also needs emergency help.

Seek prompt clinical advice for fainting, repeated near-fainting, a very slow or irregular pulse with symptoms, severe weakness, or persistent vomiting or diarrhoea while taking treatment. Sit or lie down if light-headed, rise slowly, and avoid driving until the symptom has been assessed.

Ask what target readings apply to you, when and how often to measure at home, which blood tests are required, and what missed-dose instructions apply to your exact medicine. Clarify whether another condition affects the choice, which side effects may improve, and whom to contact before making a change.
