Airborne measles: transmission, isolation and medical care

Someone who may have measles should stay away from other people and telephone a healthcare facility before attending, so staff can prevent exposure in waiting areas.

An infected person releases the measles virus while breathing, coughing or sneezing. Fine particles can remain suspended in enclosed spaces, so transmission does not require direct face-to-face contact or touching the same object. Crowded rooms and poorly ventilated indoor settings make exposure easier.

Measles commonly presents first with fever, cough, runny nose and red, watery eyes; a spreading rash follows. Small spots may appear inside the mouth. The person can pass on the virus before the rash makes the illness obvious, which is why early telephone advice and separation from others matter.

Contact a GP or public health service promptly and explain when and where the exposure occurred, whether symptoms are present, and your vaccination history. Do not enter a clinic, pharmacy or emergency waiting room without calling ahead unless immediate lifesaving care is needed. Staff can arrange a safer route for assessment.

Stay home and avoid visitors while awaiting instructions if measles is suspected. Keep away from babies, pregnant people and anyone with a weakened immune system. Use a separate, well-ventilated room when practical, avoid sharing cups or towels, cover coughs and wash hands. A clinician can decide whether vaccination or another preventive measure is appropriate after exposure.

Seek urgent help for difficult or unusually fast breathing, blue or grey lips, severe dehydration, marked drowsiness, confusion, a seizure, a stiff neck, or a rash that does not fade when pressed. Tell emergency staff about suspected measles before arrival whenever possible so they can limit airborne exposure.

Early medical advice is also needed when the affected person is a baby, pregnant, immunocompromised or unable to drink enough fluid. Measles can involve the lungs, brain or other organs, so worsening illness should not be managed solely through home remedies.

Ask whether the exposure meets the local definition of close contact, how long you should avoid others, and where testing can be arranged without exposing other patients. Confirm which symptoms should trigger another call and whether household members need individual advice.

Bring any available vaccination record and ask whether your MMR doses are up to date. If the exposed person is pregnant, immunocompromised or too young for routine vaccination, mention this at the start of the call because the available preventive approach may differ.
