Age Spots on Lips: What to Know

Short answer
Age spots on the lips are often sun-related pigment changes, but a new, changing, or unusual mark should be examined before treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical appearance
- A flat tan or brown mark with a defined edge
- First step
- Confirm the diagnosis before trying to remove pigment
- Daily care
- Protect the lips from ultraviolet exposure and monitor for change
The short answer
A typical solar lentigo is a flat, clearly outlined tan or brown patch that develops after repeated ultraviolet exposure. Pigmented lip marks can have other causes, however, and appearance alone may not establish the diagnosis.
What a pigmented lip spot may mean
Sun-related lentigines may occur on the outer lip, especially where the lip receives regular sunlight. Freckles, irritation after inflammation, medication-related pigmentation, and naturally occurring pigment patterns can look similar. A clinician may also need to exclude an atypical mole or a form of skin cancer when a mark has concerning features.
Location matters. Pigment on the dry outer lip is assessed differently from a mark on the moist inner lip or elsewhere inside the mouth. During an examination, a dermatologist considers the border, colour pattern, surface, symptoms, recent change, sun exposure, medicines, and whether similar marks appear elsewhere.
Protect and monitor your lips
Use a broad-spectrum lip sunscreen, reapply it after eating or drinking, and add shade or a wide-brimmed hat during prolonged outdoor exposure. Photograph the mark in consistent lighting so that changes in size, outline, colour, or surface are easier to notice. Avoid picking, bleaching products, acid treatments, or home freezing on lip tissue.
Do not choose a cosmetic procedure until the spot has been identified. If it is confirmed as benign, a dermatologist can explain whether leaving it alone, a carefully selected light or laser procedure, cryotherapy, or another approach suits its depth and exact location. Lip skin can react with irritation, scarring, or lighter or darker pigment, so treatment choice and aftercare need individual planning.
Changes that need medical care
Arrange a dermatology assessment for a spot that is new, enlarging, uneven in shape, varied in colour, raised, persistently rough, or different from your other marks. Prompt assessment is also appropriate if it bleeds without a clear injury, repeatedly crusts, becomes painful, forms a sore that does not heal, or causes numbness.
A clinician may inspect the area with magnification and compare it with nearby skin. If the diagnosis remains uncertain, a small tissue sample may be recommended before any pigment-removing treatment. This helps avoid masking a lesion that requires medical management.
Questions for your appointment
Ask what diagnosis fits the mark, whether magnified examination or a tissue sample is needed, and which changes should bring you back. If removal is optional, ask how the lip location and your skin tone affect the chance of irritation, scarring, or pigment change, what aftercare involves, and whether recurrence is possible with further sun exposure.

Questions people ask
Are age spots on the lips harmful?
A confirmed solar lentigo is benign, but not every brown lip mark is an age spot. Seek an examination when the diagnosis is uncertain or the mark is new, changing, symptomatic, or unlike your other spots.
Can a lip age spot be removed?
Some confirmed benign spots can be lightened or removed, but the suitable method depends on the diagnosis, pigment depth, exact lip location, and skin tone. A dermatologist should assess the mark before a cosmetic procedure.
Will lip balm prevent age spots?
A broad-spectrum sunscreen lip product can reduce further ultraviolet exposure, but it cannot reliably erase pigment that is already present. Reapplication and physical shade support ongoing protection.
