Age spots on the head: scalp and exposed-skin guidance

Solar lentigines may also appear at the hairline, on the forehead, or on the ears. Hair can hide lesions and make comparison difficult, so deliberate scalp checks are useful.

A typical age spot is a flat, smooth area of even tan-to-brown pigment. Scalp marks can instead be related to healed folliculitis, scratching, seborrhoeic growths, moles, or rough sun-related lesions, each of which may require different management.

Good lighting and parted hair help reveal the outline and surface. Ask someone you trust to look at areas you cannot see, or have the scalp examined professionally if a mark is difficult to monitor. A visible change matters more than whether a lesion sits beneath hair.

A closely woven hat provides practical cover for the crown, temples, and ears. Apply broad-spectrum sunscreen to exposed scalp, the hairline, and the tops and backs of the ears; choose a formulation you can distribute without leaving missed strips between the hair.

If a confirmed age spot is unwanted, a dermatologist can discuss freezing, a peel, or a suitable light- or laser-based treatment. Hair density, lesion thickness, healing skin, and pigment-change risk all influence selection. Avoid burning or scraping a scalp mark, and keep hair dye or fragranced products away from recently treated skin until it has recovered.

Request an examination if the mark grows, becomes asymmetric, develops mixed colours, thickens, scales, forms a recurring scab, bleeds, or feels persistently sore or itchy. A new lump or a wound that remains open also needs medical attention.

Prompt review is sensible for rapid change or repeated bleeding. A barber or hairdresser may notice a mark, but only a qualified clinician can diagnose it; postpone cosmetic removal until that assessment is complete.

Ask whether the lesion is a solar lentigo or a different growth, how you can photograph or monitor its hidden location, and whether the rest of the scalp and ears should be examined. Before treatment, discuss hair care, wound access, sun avoidance, and signs of infection or delayed healing.
