Age Spot on the Scalp: What to Check and What to Do

Age spots, also called solar lentigines, are flat areas of extra pigment associated with accumulated ultraviolet exposure. They can develop where hair is thin, the parting stays exposed, or the scalp is bald. A typical sun spot tends to have a clear edge and a fairly even tan-to-brown colour.

Appearance alone cannot confirm that a scalp mark is an age spot. A mole, irritated growth, or another skin condition can look similar, and hair may hide part of its border. A clinician can inspect the entire lesion and use magnification when needed.

Take a clear photograph with the hair parted and note whether the mark changes in size, outline, colour, thickness, or sensation. Do not scrape it or apply strong acids, bleach, or wart products. A broad-brimmed hat and sunscreen suitable for exposed scalp skin help limit further ultraviolet exposure.

If the diagnosis is confirmed and the spot is bothersome, a dermatologist may discuss targeted freezing, pigment-directed light or laser treatment, or another approach suited to the scalp. Treatment choice depends on the mark, skin tone, hair coverage, and the risk of irritation or pigment change.

Arrange a prompt skin assessment if the spot is new, enlarging, asymmetrical, irregular at the edge, varied in colour, raised, persistently itchy, painful, crusting, or bleeding. Seek care for a sore that does not heal. These features do not establish a diagnosis, but they should not be covered up or treated cosmetically first.

Ask what features support the diagnosis, whether the rest of your scalp and sun-exposed skin should be checked, and whether sampling is needed before removal. If treatment is appropriate, ask about wound care, temporary redness, possible hair disruption, and light or dark marks afterward.
