What to do after spotting and taking a pregnancy test

A test result cannot identify why bleeding occurred or confirm where a pregnancy is developing. The amount of bleeding, pain, timing and whether symptoms are changing all affect the next step.

Light pink, red or brown spotting can occur in early pregnancy, but bleeding may also accompany pregnancy loss, infection or an ectopic pregnancy. A positive home test shows that pregnancy hormone was detected; it does not show the pregnancy's location or whether it is progressing as expected.

A negative result may be reliable, or testing may have happened too early for the hormone level to register. Bleeding outside an expected period also has non-pregnancy causes, including changes at the cervix, infection, polyps or fibroids. Assessment is based on the full symptom pattern rather than spotting alone.

Note the test result, when the spotting began, its colour and whether it is getting heavier. Use a pad rather than a tampon so you can monitor the flow. If the test is positive, tell your obstetric, fertility or primary-care team about the bleeding and follow its advice on assessment.

If the result is negative but your period remains late, repeat the test according to the kit instructions or arrange a clinical test. Seek an appointment if spotting persists, returns, occurs after sex, smells unusual or comes with pelvic discomfort, fever, pain when urinating or unusual discharge.

Get urgent medical assessment if you might be pregnant and have spotting with lower abdominal or pelvic pain, especially pain on one side. Do not wait for another home test if symptoms are worsening.

Go to an emergency department or call local emergency services for heavy bleeding, severe abdominal pain, shoulder-tip pain, marked dizziness, fainting, collapse or feeling very unwell. These warning signs can occur with internal bleeding and require prompt evaluation regardless of how faint the test line looks.

Ask whether you need a repeat pregnancy hormone test, an ultrasound or tests for infection. Also ask what changes in bleeding or pain should send you directly to emergency care, and when to recheck if the first assessment is inconclusive. Bring the date of your last period, your test details, current medicines and any history of ectopic pregnancy or fertility treatment.
