The timing, location, severity, duration, and accompanying symptoms matter more than the calendar alone. New intense pain or pain with bleeding, dizziness, fever, or vomiting needs timely assessment.
Possible explanations
Ovulation discomfort is often felt on one side of the lower abdomen near the middle of a cycle and generally settles. Cramps later in the cycle may be linked to hormonal changes before a period. An ovarian cyst can also cause one-sided pressure or pain.
Not all pain in this area comes from reproductive organs. Bloating, constipation, gastroenteritis, food poisoning, gastritis, gallstones, and urinary infection can cause abdominal discomfort. If pregnancy is possible, pain with a late period or vaginal bleeding requires evaluation because an ectopic pregnancy must be excluded.
