When to get a tetanus shot after an injury

Tetanus bacteria can enter through broken skin, particularly deep punctures and wounds contaminated with soil, dust, manure or saliva. Crush injuries, burns and wounds containing dead tissue also need careful assessment. A clinician combines the wound type with your vaccination record; the presence or absence of rust is not the deciding factor.

A booster may be advised if protection is not current. People with an incomplete or uncertain vaccine history may need a vaccine dose and, for certain high-risk wounds, tetanus immune globulin for immediate short-term protection. Vaccination prevents future illness but does not replace cleaning, removal of debris or treatment of infection.

Rinse the wound under clean running water, wash the surrounding skin and remove loose visible dirt with clean tweezers if this can be done easily. Apply gentle pressure to bleeding with clean material, then cover the area. Do not probe a deep wound or seal contamination inside it. An animal or human bite, embedded object, puncture, burn or heavily soiled wound warrants prompt professional care.

Look for a vaccination record, but do not delay care if it cannot be found. Tell the clinician when the injury happened, how it occurred, what contaminated it, whether anything remains inside and whether immune-suppressing illness or treatment affects you.

Get urgent help for bleeding that will not stop, a deep or gaping wound, loss of feeling or movement, an embedded object, major contamination, or an injury involving the eye, face, hand or a joint. Increasing redness, warmth, swelling, pus, worsening pain or fever can indicate a wound infection and needs timely review.

Jaw stiffness, painful muscle spasms, trouble swallowing or breathing, or marked body rigidity after a wound are emergency symptoms. Do not wait for these signs before checking vaccination status: tetanus prevention is most useful when addressed promptly after the injury.

Ask whether the wound is considered clean or tetanus-prone, whether your documented doses provide current protection, and whether you need a booster or immune globulin.
