Treatment for advanced prostate cancer

The choice and sequence depend on where the cancer is, whether it still responds to hormone lowering, previous therapies, symptoms, other health conditions, and what matters most to you.

Hormone-lowering therapy is often a foundation of care because prostate cancer commonly uses male hormones as a growth signal. Additional medicines may block that signalling more fully, damage dividing cancer cells, or act on particular features found through tumour or inherited-gene testing.

Radiotherapy can treat a troublesome site or, in selected circumstances, form part of broader disease control. Targeted radioisotope therapy carries radiation to cancer cells with suitable biological features. Bone-strengthening treatment may be considered when cancer or prolonged hormone lowering puts skeletal health at risk.

Before agreeing to a plan, ask about its aim, how it is given, how response will be checked, likely side effects, and what alternatives exist. Share all prescribed medicines, supplements, allergies, dental concerns, and medical conditions because these details can affect treatment safety.

Keep appointments for blood tests and imaging, and record side effects between visits. Exercise, nutrition advice, pain control, emotional support, sexual-health care, and management of urinary symptoms can remain part of treatment rather than being postponed until cancer-directed therapy ends.

Follow the oncology service’s urgent instructions for fever, shaking chills, unusual bleeding, shortness of breath, chest pain, severe diarrhoea or vomiting, confusion, or inability to drink. Some treatment-related problems need assessment before the next scheduled visit.

Seek emergency help for new leg weakness, numbness around the groin, loss of bladder or bowel control, or severe new back pain. Inability to pass urine and a sudden painful limb that cannot bear weight also require urgent evaluation.

Ask why a particular treatment is recommended now, which benefit matters most in your situation, how it could affect energy, bone health, sexual function, or daily independence, and what would lead to a change of plan. Clarify who to call after hours and which side effects should never wait.
