Symptoms of advanced prostate cancer

The pattern depends on where the cancer is and how it affects the body. Symptoms alone cannot confirm progression, because treatment effects and unrelated conditions can cause similar problems.

Cancer near the prostate can present with a weak urine flow, difficulty starting, incomplete emptying, or blood in urine. Spread to bone may present with pain that persists, becomes focused in one place, or interferes with sleep and movement. Wider effects can include low energy, reduced appetite, weight loss, or breathlessness.

A new symptom does not by itself show where the cancer is or whether it has changed. The clinician combines your history and examination with appropriate blood tests and imaging to identify the cause.

Note when a symptom began, its location, what makes it better or worse, whether it disrupts sleep or walking, and which medicine you tried. Report a persistent change rather than waiting for the next routine appointment, especially if it is worsening.

Mention changes in bladder or bowel function, falls, numbness, new swelling, eating and drinking, and ability to manage normal activities. Bring an accurate medicine list because pain medicines, hormone therapy, and other treatments can contribute to fatigue, constipation, dizziness, or appetite changes.

Seek emergency care for new weakness or heaviness in the legs, difficulty walking, numbness around the buttocks or genitals, loss of bladder or bowel control, or severe new back pain. These symptoms can reflect spinal cord compression.

Get prompt help if you cannot pass urine, have uncontrolled bleeding, become acutely confused, develop sudden severe pain, or feel very unwell with fever during cancer treatment. Use the urgent contact instructions supplied by your oncology service when available.

Ask which cause is being considered for each symptom, whether a scan or blood test is needed, how quickly treatment should help, and what to do if it does not. Request a written plan for regular pain relief, breakthrough pain, constipation prevention, and after-hours concerns.
