The short answer

There is no single laboratory panel that reliably identifies every intolerance. Depending on the history, a clinician may use a breath test for impaired digestion of a particular sugar, blood tests to check for another condition, or a structured elimination and reintroduction plan.

How to interpret food intolerance tests

Food intolerance usually describes difficulty digesting or processing a food component rather than an immune allergy. It may present with bloating, cramps, excess gas or diarrhoea after a relevant food. The timing, portion size and repeatability of symptoms help determine whether a targeted test is sensible.

Some commercial panels measure IgG antibodies against many foods. Exposure to a food can produce these antibodies without showing that the food causes symptoms, so a long list of positive results can encourage unnecessary restriction. Tests for coeliac disease or food allergy answer different questions and should not be labelled as general intolerance tests.

A safer route to an answer

Record meals, portion sizes, symptom timing, bowel changes and medicines for a short period without making several changes at once. Discuss the pattern with a GP or dietitian, who can look for infection, coeliac disease, inflammatory bowel disease, medication effects or irritable bowel syndrome before focusing on one food component.

If an elimination trial is appropriate, remove only the suspected trigger for an agreed period, plan nutritionally suitable alternatives and reintroduce it methodically. Continue eating gluten before coeliac blood testing unless your clinician instructs otherwise, because avoiding it beforehand can affect the result.

When to get medical help

Call emergency services if eating is followed by trouble breathing, throat or tongue swelling, collapse, or rapidly developing widespread hives with other symptoms. These features can indicate a serious allergic reaction, which is different from food intolerance.

Arrange prompt medical review for blood in the stool, persistent vomiting, fever, dehydration, unintentional weight loss, symptoms that wake you repeatedly, difficulty swallowing or ongoing diarrhoea. Children, pregnant people and anyone already underweight should receive professional guidance before restricting major food groups.

Questions before paying for a test

Ask what biological mechanism the test measures, whether the result changes management, what could cause a false result and whether food must remain in your diet beforehand. Clarify who will interpret the findings and help you reintroduce foods so that a test does not lead to avoidable nutritional gaps.

No. Food allergy involves an immune reaction and can cause rapid swelling, breathing difficulty or collapse. Intolerance more often causes digestive symptoms and depends on the component and amount eaten.

Can a home test tell me exactly which foods to avoid?

A result must be matched to a plausible mechanism and your symptom pattern. Broad panels may flag foods that you tolerate, so making long-term exclusions from the report alone can be misleading.

Why is reintroduction part of an elimination trial?
