The short answer
Actinic keratoses are sun-damaged, precancerous skin growths rather than invasive cancer. Some can develop into squamous cell carcinoma, and appearance alone cannot reliably identify which ones will change.
What “advanced” can mean
Actinic keratosis does not have a standard advanced stage. Clinicians may use the word informally for many lesions across a sun-damaged area, a thick or tender growth, patches that repeatedly return, or disease in someone whose immune system is suppressed.
A dermatologist examines the individual spots and the surrounding skin. A lesion that is unusually thick, growing, bleeding, ulcerated or persistently painful may be sampled with a biopsy to distinguish actinic keratosis from squamous cell carcinoma or another condition.
What to do next
Arrange a full skin assessment and note which areas are new, changing or symptomatic. Treatment may target a single lesion with freezing or removal, or treat a wider field of damaged skin with a prescription cream or photodynamic therapy. The choice depends on the site, thickness, number of lesions, previous treatment, general health and ability to manage the expected skin reaction.
Use prescribed medicine only on the area and schedule given by your clinician. Redness, crusting and soreness can be expected with some field treatments, but contact the treating team if the reaction is difficult to manage or looks infected. Protect exposed skin with shade, clothing and broad-spectrum sunscreen, and avoid tanning devices.
When to seek care promptly
Request a prompt dermatology review for a patch that becomes a firm lump, enlarges, bleeds without clear injury, forms an open sore, changes rapidly or becomes persistently painful. These features need examination because squamous cell carcinoma can present with them; they do not establish the diagnosis by themselves.
Seek earlier review if lesions appear on the lips, if numerous new rough areas develop, or if your immune system is suppressed by illness or medication. After treatment, return if a treated site does not heal as expected or the roughness comes back.
