Adult Tetanus Booster: What You Should Know

Tetanus is a serious infection caused by bacteria that can enter through broken skin. A booster refreshes the immune response created by earlier tetanus vaccination; it does not treat an infection that is already developing. Adult protection is often provided as part of a combined vaccine, so the product may also cover another infection.

The need for a booster depends on your previous doses, when the last dose was given, and whether the wound is clean and minor or deeper, dirty, punctured, or contaminated. If your vaccination history is uncertain, tell the clinician rather than guessing. An inactivated vaccine is used for this type of protection.

Rinse the wound with clean running water, remove visible dirt only if it comes away easily, and cover it with a clean dressing. Do not put household substances into a deep wound. Arrange medical advice promptly if the injury is punctured, heavily contaminated, caused by a bite, or difficult to clean. Share the date of your last tetanus-containing vaccine and any record of earlier doses.

A clinician may clean the wound, assess its depth and contamination, and decide whether a booster is appropriate. Wound suturing may be considered after assessment and cleaning, but closing a wound does not replace tetanus prevention.

Seek urgent medical care for trouble opening the mouth, painful muscle stiffness or spasms, difficulty swallowing, breathing difficulty, or rapidly worsening symptoms after a wound. These symptoms need prompt assessment and are not a reason to wait for a routine vaccination appointment.

Get prompt advice for a deep or puncture wound, a wound containing soil or a foreign object, or an injury with increasing redness, swelling, warmth, drainage, or fever. People with conditions or medicines that weaken immune responses should mention this when seeking advice.

What wound changes should prompt a review? Tell the clinician about vaccine allergies, a previous severe reaction, pregnancy, immune-suppressing treatment, and medicines you take.
