Change wet or soiled absorbent products promptly and avoid rubbing already inflamed skin. A persistent rash may instead involve yeast, bacteria, allergy, pressure damage or another skin disorder, so treatment should match the cause rather than relying on repeated over-the-counter creams.

Urine, stool, sweat and friction can weaken the skin barrier beneath an incontinence pad or brief. This is often called incontinence-associated dermatitis. It tends to affect exposed surfaces and folds differently depending on where moisture collects, and it can burn or feel tender.

Yeast may complicate damp skin and can produce a bright rash that reaches into folds with smaller spots nearby. A sharply outlined reaction may follow a wipe, cleanser or adhesive. Pressure injury is another concern over bony areas, especially when movement is limited, and requires a different care plan.

After each episode of urine or stool exposure, cleanse with lukewarm water or a mild no-rinse product and pat dry. Avoid fragranced wipes, talcum powder, hot water and vigorous scrubbing. Allow air to reach the skin when practical, and use a breathable, correctly fitted absorbent product to reduce rubbing.

Apply a thin, even layer of a barrier product suitable for incontinence care; removing every trace at each change can cause extra friction. Address leaking, diarrhoea and fit problems with a clinician or continence professional. If yeast is suspected, ask a pharmacist or doctor whether topical antifungal treatment is appropriate instead of choosing it from appearance alone.

Get prompt advice for broken skin, ulcers, blisters, pus, spreading warmth or redness, increasing pain, fever or a person who seems generally unwell. Dark purple or black discoloration, rapidly worsening tissue damage or severe pain needs urgent assessment.

A rash that persists despite careful moisture and barrier care should be examined. Earlier review is sensible for someone with diabetes, poor circulation, reduced immunity, limited mobility or difficulty communicating pain, because infection and pressure injury may be harder to recognize.

Ask whether the pattern is irritant dermatitis, yeast, allergy or pressure damage; which cleanser and barrier are compatible; and how often the skin should be checked. Bring the names of current pads, wipes and creams so possible irritants and product fit can be reviewed.
