Adult buttock rash: causes, self-care and warning signs

Its position and appearance help narrow the possibilities. A rash deep in the crease behaves differently from scaly rings, grouped painful blisters, itchy bumps or plaques elsewhere on the buttocks. Because different causes need different treatments, avoid assuming every rash is fungal or treating repeatedly without a diagnosis.

Moisture and skin rubbing can produce intertrigo in the cleft or where skin surfaces meet. Contact dermatitis can follow fragranced wipes, cleansers, laundry products, adhesives or topical medicines. Heat rash tends to appear after heavy sweating and occlusion. Ringworm often forms an enlarging scaly edge, while scabies commonly causes intense itching and may affect household contacts.

Psoriasis can create sharply defined plaques in or around the cleft, and atopic dermatitis may present with itchy, inflamed skin in people prone to eczema. A widespread eruption after starting a medicine can be a drug rash. These conditions can overlap in appearance, so photographs of the early rash and a timeline are useful during assessment.

Wash gently with lukewarm water and an unperfumed cleanser, then pat rather than rub the area dry. Change out of sweaty clothing, choose breathable loose garments and pause new wipes, fragranced products or hair-removal methods. Do not share towels when an infection is possible, and avoid scratching or squeezing bumps.

Note where the rash began, whether it itches or hurts, and whether there is scale, drainage, blisters or a sharply defined border. List new medicines and skin products. A pharmacist or clinician can then match treatment to the likely cause; steroid cream used on an unrecognised fungal infection, for example, may alter its appearance and allow it to spread.

Get prompt assessment if the area is rapidly spreading, very painful, hot and swollen, producing pus, or accompanied by fever or feeling seriously unwell. Painful grouped blisters, an open sore near the anus or genitals, and a rash that began after a new medicine also deserve timely clinical advice. People with reduced immunity or poorly controlled diabetes should seek help early for signs of infection.

Call emergency services for breathing difficulty, facial or throat swelling, faintness, or a widespread blistering or peeling eruption. Book a routine visit if careful skin care does not settle the problem, it keeps returning, other household members itch, or the diagnosis remains unclear.

Ask whether the distribution suggests irritation, eczema, psoriasis, fungus, scabies or another infection, and whether a skin scraping or swab would clarify the cause. Confirm exactly where and how to apply any medicine, which household or hygiene measures are necessary, and what change should trigger an earlier review.
