ADT drugs in prostate cancer

ADT drugs lower testosterone production or block androgen signals that can help prostate cancer cells grow. Treatment may involve an injection, an implant, a tablet, or a combination. The specific medicine depends on the cancer's stage and response, other health conditions, and which additional cancer treatment is planned.

GnRH agonists reduce testosterone after a brief initial rise, sometimes called a flare. GnRH antagonists lower testosterone without that initial rise. Androgen-receptor blockers interfere with the signal inside cancer cells, while androgen-synthesis inhibitors reduce androgen production beyond the testicles. These medicines are not interchangeable; some are used alone and others are added to ongoing testosterone-lowering treatment.

ADT may be paired with radiotherapy for selected localised or locally advanced disease and is commonly part of treatment when prostate cancer has spread. Blood tests, symptoms, scans, and treatment tolerance help the oncology team judge whether the plan is controlling the cancer.

Bring a complete medicine and supplement list to the prescribing visit because interactions can affect which tablet is suitable. Confirm how each dose is taken, whether food instructions apply, and what to do after a missed dose. Keep injection appointments and planned blood tests even when you feel well.

Regular weight-bearing activity and strength work, adapted to your fitness and bone health, can help protect muscle and mobility. Tell the team early about hot flushes, fatigue, mood changes, breast discomfort, sexual difficulties, or weight change; supportive options can often make treatment easier to continue.

Contact the cancer team urgently for new weakness or numbness in the legs, loss of bladder or bowel control, severe or rapidly worsening back pain, inability to pass urine, chest pain, fainting, or sudden breathlessness. These symptoms need assessment for complications such as spinal cord pressure, urinary obstruction, or a heart or circulation problem.

A severe rash, swelling of the face or throat, or trouble breathing after a dose may be an allergic reaction and requires emergency help. For less urgent but persistent effects, call the clinic rather than stopping ADT on your own.

Ask which drug class you are receiving, why it fits your cancer, whether another cancer treatment will be combined with it, and how response will be checked. Also discuss bone health, heart and metabolic health, sexual wellbeing, fertility if relevant, expected administration schedule, and who to call between appointments.
