Using an adrenaline auto-injector pen

Different brands operate differently, so use the instructions printed on the prescribed device rather than relying on a generic sequence. After any injection, call local emergency services because the pen begins emergency treatment but does not replace medical assessment.

The device holds a measured dose of adrenaline and is designed to deliver it quickly into the outer mid-thigh. Adrenaline can reduce airway swelling, relax tightened airways and support circulation during anaphylaxis. The correct device strength is selected by a clinician for the individual.

Warning signs can include throat or tongue swelling, a hoarse voice, noisy or difficult breathing, wheezing, faintness or collapse. Skin changes such as hives or flushing may occur, but their absence does not rule out anaphylaxis.

Remove the safety mechanism only when ready to inject. Hold the pen as its label shows, keep fingers away from either end, press the designated end firmly against the outer mid-thigh and maintain contact for the time stated in its instructions. It can usually be given through ordinary clothing, but avoid seams or bulky items in the way.

Store the pen where it is readily accessible, in its protective case and away from excessive heat, cold or direct sunlight. Do not refrigerate it. Check the expiry date and viewing window regularly, replace it before expiry, and seek a replacement after use or if the solution is discoloured or contains particles.

Call local emergency services immediately and explain that adrenaline was used for suspected anaphylaxis. Keep the person lying flat with legs raised if tolerated; someone struggling to breathe may sit with legs extended. Do not allow sudden standing or walking, which can worsen a dangerous drop in blood pressure.

If severe symptoms continue or return and another prescribed auto-injector is available, follow the person's written emergency plan and the device directions. Start resuscitation if the person is unresponsive and not breathing normally. Medical observation remains necessary even after an apparent recovery because symptoms can recur.

Ask your clinician to demonstrate your exact model with a trainer and watch you practise. Clarify which symptoms should prompt use, where your pens should be kept, whether you should have access to more than one device, and what carers, relatives, colleagues or school staff need to know.

Review possible allergy triggers and whether specialist assessment is appropriate. Confirm how to obtain replacements, how your specific pen signals that it has activated, and what to do if an injection is accidentally given into a finger or hand; accidental injection there needs prompt medical advice.
