ADHD and sexually inappropriate behaviour

Behaviour may include sexual comments, exposing private areas, unwanted touching, repeated messaging, sharing intimate material or ignoring another person's refusal. The immediate priorities are clear boundaries, protection of anyone affected and an assessment of why the behaviour occurred.

Impulsivity can reduce the pause between an urge and an action, while difficulty reading a situation may lead to missed social cues. Other possibilities include limited education about consent, developmental differences, mania, substance use, trauma, exposure to sexual content, coercion or abuse. New behaviour that is markedly out of character needs medical attention.

Assessment should consider age, developmental level, context, intention, repetition, power differences and the impact on others. Consent must be freely given, specific, informed and reversible; silence, freezing, intoxication or previous consent is not permission for a current act.

Stop the behaviour calmly and directly, separate people when needed, and state the boundary in concrete language. Check privately on the person affected without pressuring them to confront anyone. Record what happened in factual terms, including words and actions, and follow the safeguarding procedure for the setting.

For prevention, teach explicit rules about privacy, personal space, consent, digital sharing and how to respond to a refusal. Rehearse acceptable alternatives, supervise according to risk and reduce access to situations where harm could recur. Treatment may involve behaviour planning, relationship or sexuality education, therapy and a review of ADHD or other mental health care.

Contact emergency services if anyone is in immediate danger, there has been sexual assault, threats or serious violence, or the person cannot control an imminent urge to harm someone. Move to safety and preserve messages or other potential evidence without circulating intimate material.

Seek prompt safeguarding and clinical advice when a child is involved, there is a power imbalance, behaviour is repeated or escalating, coercion is suspected, or the conduct began suddenly with reduced sleep, extreme energy, confusion or major personality change. Local reporting duties may apply to professionals and carers.

Ask which factors may be driving the behaviour, whether another mental health or developmental condition needs assessment, and what supervision is appropriate now. Request a written plan that defines boundaries, early warning signs, safe responses and how the needs of the person affected will be protected.
