ADHD and seizures: assessment, treatment and safety

A person may have both conditions, one condition, or another explanation for an episode. ADHD does not make every lapse in attention a seizure, and a seizure cannot be identified from concentration problems alone.

ADHD-related inattention often varies with interest, fatigue and the environment, and the person may respond when redirected. Some seizures can cause a sudden interruption of awareness, repeated automatic movements, stiffening, jerking or confusion afterward. Not every seizure looks like a convulsion, and daydreaming alone is not enough to diagnose epilepsy.

A clinician needs an eyewitness description, the sequence of the event, its duration, awareness and recovery. A safely recorded video may be useful. Medical history, examination and specialist tests are chosen according to the event; no single symptom settles the diagnosis.

Tell both the ADHD prescriber and seizure specialist about every medicine, supplement and missed dose. Do not start, stop or alter either treatment independently. The clinicians can consider seizure control, attention symptoms, sleep, side effects and interactions when building a coordinated plan.

During a convulsive seizure, protect the head, clear nearby hazards, loosen tight clothing around the neck and time the event. Do not restrain the person or place anything in their mouth. When jerking stops, roll them onto their side if you can do so safely, check breathing and remain with them as awareness returns.

Call local emergency services for a first suspected seizure, an event lasting five minutes or longer, repeated seizures without recovery, serious injury, breathing difficulty, a seizure in water, or a seizure during pregnancy. Follow an existing emergency care plan if the person has one.

Seek prompt clinical advice for new staring episodes, unexplained collapses, waking with injuries, unusual movements or confusion after an event. Until assessed, avoid situations where a sudden loss of awareness could cause serious harm, and ask about local rules for driving and hazardous work.

Ask which details would help classify an episode, whether referral or testing is needed, and how each medicine affects the overall plan. Request written seizure first-aid instructions for family, school or work and confirm whom to contact if events or side effects change.
