ADHD and bipolar disorder

Short answer
ADHD and bipolar disorder can share some features, but their timing and pattern help distinguish them.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- ADHD course
- A developmental pattern that usually traces back to childhood.
- Bipolar course
- Distinct mood episodes with a change from usual functioning.
- Key record
- A timeline of mood, energy, sleep and behaviour changes.
The short answer
ADHD commonly presents as a longstanding developmental pattern of attention-regulation difficulty, impulsivity and restlessness. Bipolar disorder presents with distinct mood episodes that involve a marked change from usual functioning. During mania or hypomania, elevated or irritable mood may occur with reduced need for sleep, increased energy, rapid speech, racing thoughts or risky behaviour.
How clinicians separate overlapping features
Distractibility, fast speech, agitation and impulsive choices can appear in either condition. ADHD features generally trace back to childhood and remain relatively consistent, although their impact varies by setting. Bipolar symptoms cluster into episodes, with a noticeable departure from the person's baseline mood, energy, sleep and behaviour.
Both conditions can occur in the same person. The assessment therefore maps symptoms over time and considers depression, anxiety, trauma, substance use, sleep disruption, thyroid or other physical illness, and medicine effects. Accurate identification matters because treatment choices and monitoring differ.
Building a clear timeline
Record changes in sleep, energy, mood, speech, spending, risk-taking, concentration and daily functioning. Note when each change started, whether others noticed it and whether symptoms returned to the usual baseline. Include childhood attention or behaviour records and a complete list of medicines, supplements, caffeine, alcohol and other substances.
Do not start, stop or change psychiatric medicine without the prescriber. If ADHD treatment is being considered in someone with possible bipolar disorder, the clinician should review the mood history and plan monitoring. Regular sleep, reduced substance use and involvement of a trusted person can help reveal early changes while professional care is arranged.
Mood changes that need urgent help
Seek urgent local assessment for thoughts of suicide or harm, hallucinations, severe confusion, dangerous risk-taking, aggression, inability to care for basic needs, or a striking mood and energy change with very little sleep. If immediate danger is present, stay with the person when safe, reduce access to hazards and contact emergency services.
Questions that sharpen the diagnosis
Ask whether the symptoms are continuous or episodic, what signs would suggest mania or depression, and which physical or substance-related causes need checking. Clarify the treatment sequence if both diagnoses are possible, which early warning signs to track, and whom to contact if sleep, mood or behaviour changes quickly.

Questions people ask
Does irritability show whether someone has ADHD or bipolar disorder?
No. ADHD and bipolar disorder can both present with irritability. Its timing, accompanying changes, duration and relationship to the person's baseline provide more useful clues.
Can someone have both ADHD and bipolar disorder?
Yes. A clinician can evaluate both, using developmental history and a mood timeline to understand which features belong to each condition.
Why does sleep matter during assessment?
Reduced need for sleep with increased energy can accompany a bipolar mood episode, while insufficient sleep can also worsen attention and emotional control for many reasons.
