Adenocarcinoma and small cell lung cancer explained

Adenocarcinoma is generally classified as non-small cell lung cancer, while small cell lung cancer has different microscopic features and behaviour. Occasionally, a tumour contains more than one pattern; the pathology report should state the exact diagnosis.

The name comes from cells examined under a microscope. Adenocarcinoma forms gland-like structures or produces mucus. Small cell cancer is made of smaller-appearing neuroendocrine cells and can spread early. A report may use the term combined small cell carcinoma when small cell features occur with another lung cancer component.

Treatment planning also depends on where the cancer is, whether it has reached lymph nodes or other organs, and the person's breathing, general health and preferences. The diagnostic label alone does not provide the full plan.

Ask for the complete pathology wording and whether a specialist lung pathologist has reviewed an unusual or mixed result. Imaging and tissue findings are considered together to establish the type and extent of disease.

Depending on the confirmed diagnosis and stage, the care team may discuss chemotherapy, radiotherapy or immunotherapy. A treatment used for one lung cancer subtype may not be appropriate for another, so clarification should come before decisions.

Seek urgent medical assessment for coughing up more than a small streak of blood, severe or suddenly worsening breathlessness, new confusion, a seizure, fainting, or new weakness on one side. Contact the oncology team promptly for fever during cancer treatment, rapidly increasing pain, or symptoms that prevent drinking or taking medicines.

Useful questions include: What is the exact tumour type? Is there a mixed component? What stage has been assigned? Is more tissue testing needed? What is the goal of treatment, and which side effects need an urgent call? If brain treatment is mentioned, ask why whole-brain radiotherapy is being considered and what alternatives apply to your situation.
