How it can feel

The adductor longus helps control the thigh as the leg moves inward and during rapid changes of direction. Symptoms may appear when squeezing a ball between the knees, pushing sideways, taking a long stride or rising from a wide position.

Groin pain is shared by many problems. Hip impingement can present with groin discomfort during bending or rotation; a hip labral tear can present with pain plus catching; and hip osteoarthritis can present with groin pain and stiffness. These patterns overlap and need clinical interpretation.

Adjusting activity and rebuilding capacity

Reduce the particular actions that provoke sharp pain, such as fast lateral cuts or powerful kicks, but keep moving within a manageable range if walking is comfortable. Note what triggers symptoms and how the area responds later that day and the following morning.

Recovery work can begin with low-load adductor contractions and progress toward hip, trunk, running and change-of-direction exercises. A sports physiotherapist can test both sides and tailor progression to your work or sport demands.

Getting medical help

Seek urgent care if severe pain follows a collision or fall and you cannot stand, if the limb looks misshapen, or if the leg becomes numb, weak, pale or cold. Rapid swelling, severe abdominal pain, acute testicular pain, fever or a painful groin lump also needs prompt evaluation.

A routine assessment is appropriate when pain repeatedly returns with activity, causes limping, disturbs sleep or fails to improve after sensible load reduction. Earlier review is useful when the cause is unclear.
