Adductor brevis muscle pain: causes and next steps

Short answer

Adductor brevis pain is usually felt in the inner upper thigh or groin and may arise from overload, strain, or another nearby structure.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Deep inner upper thigh or groin

Squeezing the legs together, cutting, kicking or taking a long stride

Groin location alone does not identify the painful tissue

What adductor brevis pain may feel like

The adductor brevis is a deep inner-thigh muscle that helps draw the leg toward the body's midline and contributes to hip control. Discomfort may appear when squeezing the legs together, changing direction, kicking, taking a long stride or getting out of a low seat. Location alone cannot confirm which tissue is responsible.

Why the source is not always obvious

Pain can follow a sudden sporting movement or build gradually when training load exceeds the muscle's current capacity. Tenderness and pain against resisted adduction can support an adductor source, yet neighbouring muscles, tendons, the hip joint, the abdominal wall and referred pain can produce a similar pattern.

Hip impingement, a hip labral tear and hip osteoarthritis can each present with groin pain, but none can be identified from that symptom alone. Clicking, restricted hip rotation, stiffness, the mechanism of onset and changes during specific movements help a clinician narrow the possibilities. Examination may also include the back, pelvis and abdomen when the pattern calls for it.

What you can do initially

Reduce or modify movements that sharply reproduce the pain, particularly cutting, forceful kicking and wide lateral steps. Keep comfortable general movement rather than repeatedly testing the sore action. Cold or heat may be used for comfort if appropriate for you, but neither determines the cause or replaces rehabilitation.

Arrange an assessment if pain limits walking, sport or daily activity, follows a significant injury, or does not begin to settle. A physiotherapist can compare hip motion, adductor strength, tendon loading and movement control, then progress exercise according to symptoms. Avoid aggressive stretching into groin pain because a strained or irritated structure may not tolerate it.

When groin pain needs faster assessment

Seek urgent care after a major injury if you cannot bear weight, the hip or leg looks deformed, swelling is rapidly increasing, or the limb becomes numb, pale or cold. Sudden severe groin or testicular pain, a painful groin lump, abdominal pain with vomiting, fever, or feeling acutely unwell also requires prompt medical assessment.

Book a clinical review for pain that wakes you persistently, unexplained swelling, progressive weakness, repeated catching or locking, or symptoms spreading from the back with altered sensation. These findings broaden the assessment beyond a simple muscle overload problem.

Useful questions at an appointment

Ask which examination findings point toward the adductor brevis, whether the hip joint or another region should be assessed, and what activities are currently safe. Request clear criteria for increasing resistance, returning to running or changing direction, and ask when imaging would alter management rather than simply show incidental findings.

Questions people ask

How can I tell whether the adductor brevis is causing my pain?

You cannot confirm it reliably by touch or location alone because the muscle lies deep and several nearby structures refer pain to the groin. A clinical examination can test adductor loading and assess competing causes.

Should I stretch inner-thigh pain?

Do not force a stretch that reproduces sharp pain. Early management often favours comfortable movement and graded loading; the useful range and timing depend on whether the problem is muscle, tendon, joint or another structure.

Does groin pain mean I have a hip labral tear?

No. A hip labral tear can present with groin pain, but muscle, tendon, joint and non-musculoskeletal problems can create a similar symptom. Examination and the broader symptom pattern guide further testing.
