What should you know about acute trochanteric bursitis?
Pain may be sharper when lying on the affected side, climbing stairs, rising from a low seat, or walking for longer than usual. A clinician can check whether the bursa, nearby gluteal tendons, the hip joint, or the lower back is the main pain source.
What the diagnosis means
The trochanteric bursa is a small cushioning sac beside the bony point on the outside of the hip. Sudden overload, a direct knock, or repeated compression can irritate this region. Clinicians also use the broader term greater trochanteric pain syndrome because nearby tendons can contribute alongside the bursa.
Symptoms are usually felt on the side of the hip rather than deep in the groin. Tenderness when pressing the outer hip and pain during side-lying are useful clues, but an examination helps distinguish this problem from arthritis, tendon injury, or pain referred from the spine.
Steps to settle the hip
Temporarily reduce movements that repeatedly provoke the pain, while keeping up comfortable daily movement. Avoid sleeping directly on the sore side; when lying on the other side, a pillow between the knees can reduce compression. A wrapped cold pack may ease a recent flare, and suitable pain medicine can be discussed with a pharmacist or clinician.
As the flare calms, physiotherapy may rebuild hip strength and adjust walking, stair, or training loads. If symptoms persist, a clinician may consider a cortisone injection or shockwave therapy after confirming the pain source. These options complement load management rather than replacing it.
When to seek medical care
Arrange an assessment if pain is worsening, keeps returning, disrupts sleep despite position changes, or limits normal walking. Seek urgent care after a significant fall if you cannot bear weight, the leg looks deformed, or hip movement is severely restricted.
Prompt medical review is also needed for a hot, red, markedly swollen hip area, fever, feeling acutely unwell, new leg weakness or numbness, or loss of bladder or bowel control. These features require evaluation for causes beyond uncomplicated bursal irritation.
