Acute paronychia on a finger

It commonly begins after a hangnail, cuticle injury, nail biting, finger sucking, manicure or small puncture. Pus may collect beside or beneath the nail fold, and gripping or pressing with that finger can become uncomfortable.

Cuticle damage opens the narrow barrier that normally keeps microbes out. Frequent wet work, detergents and repeated picking can prevent that barrier from closing. Sudden one-sided pain and swelling suit an acute nail-fold infection, while recurrent irritation across several nails may have another cause.

A clinician looks for fluctuance or a pale centre that suggests fluid under pressure. Grouped clear blisters, pain extending into the fingertip pad, or swelling around a joint can point away from straightforward paronychia and should not be opened at home.

Soak a mild case in comfortably warm water at intervals through the day and dry the finger fully afterward. Remove rings early if swelling is developing, avoid cuticle tools and biting, and use gloves for wet tasks without leaving the hand sweaty inside them.

Do not force a needle under the nail fold. Treatment may involve prescribed topical care, oral antibiotics when infection is more extensive, or clinician-performed drainage when pus has formed. Continue wound care exactly as directed if the area is opened.

Book prompt care when a pus pocket appears, tenderness intensifies, redness travels up the finger, movement becomes limited or the swelling fails to settle. Fever, red streaks along the hand or arm, marked fingertip-pad pain, loss of sensation or a trapped ring require urgent attention.

People with diabetes, poor hand circulation or reduced immune defences should seek advice early. Tell the clinician about splinters, bites, punctures and contact with mouth secretions because those details can affect the likely organisms and medicine choice.

You can ask whether pus is present, whether the nail itself is involved and how to dress the finger while working. Also clarify when normal hand use can resume and which changes should bring you back for reassessment.
